GENERAL INFORMATION:

The City of Chino Hills (“City”) invites your organization to submit a written proposal to provide on-call services for asphalt and concrete repair. The successful proposer will be awarded a  "not to exceed amount" maintenance contract for a five-year period. The proposal must clearly demonstrate how your organization would best satisfy the requirements of the City. This written Request for Proposal (RFP) states the scope of the City’s requirements and specifies the general rules for preparing the proposal.

Special Requirements:
The proposer must submit a package clearly marked on the outside as “Proposal for On-Call Asphalt Repair” to the City Clerk’s Office. The package must contain the following:
• Five (5) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal;
• One (1) original and four (4) copies of the cost proposal in a separate sealed envelope clearly marked “Sealed Cost Proposal”; and
• A signed W-9.
