**Addendum No.1 - RFP Engr Design Services-CP Parking Lot, dated 6/01/2023

The following is a summary only and does not replace the full RFP.
 
Background:

The City of Chino Hills is seeking a qualified firm to perform engineering design services associated with the Community Park Expansion of Parking Lot and Pavement Improvements Project.  The City is considering improving the two areas of the Community Park to expand the existing parking lot (see Attachment No. 1-Location Map) into the following phases:

1.  Phase A - Parking Lot Expansion at open grass area on the west parking lot (see Attachment No.2 - Phase A Conceptual Parking Lot Design Layout-West End). Expansion will add approximately 82 parking spaces.

2.  Phase B - Parking Lot Expansion at the sloped grass area between the two Park Entrances and south of the parking lot (see Attachment No. 3 - Phase B Conceptual Parking Lot Design Layout-Southwest Side).  Expansion will add approximately 84 parking spaces.

In addition, the project will include other Community Park improvements including reconstruction of the existing trash enclosure, and rehabilitation and improvements of various pavement surfaces (i.e., including repair of sidewalks, walkways, access ramps, and ac pavement) within the entire Community Park.

The submittal package shall include: five (5) bound copies and one (1) electronic copy (flash drive is preferred method, emails not accepted) of the proposal, one (1) original and four (4) copies of the cost proposal in a separate sealed envelope clearly marked "Sealed Cost Proposal" and a signed W-9.

Proposer must be willing to enter into an agreement with the City of Chino Hills. A copy of the agreement accompanies the full RFP. Please review this agreement and provide the City with a written statement of your firm's willingness to accept the terms of the agreement. Please specifically identify each and every term of the agreement which your firm is unwilling to accept and the reason therefore.
