Passport Services

The City of Chino Hills City Clerk's Office serves as a Passport Acceptance Facility. Appointments are required for all services, walk-ins are NOT accepted. Use the passports button below to schedule an appointment if you are applying for the first time or have lost or damaged your passport.

Hours & Location

Passport appointments are processed in the City Clerk's Office on the second floor of City Hall, located at 14000 City Center Drive, Chino Hills, during the following hours:

  • Monday and Wednesday, 8:00 am to 5:00 pm
  • Tuesday, 9:00 to 4:00 pm
  • Thursday, 9:00 am to 5:00 pm
  • Friday, 8:00 am to 12:00 pm

Here's What You'll Need

All applicants are required to bring the following items to their appointment:

  1. A Completed Passport Application (Form DS-11)
  2. Proof of U.S. Citizenship, such as: 
    • Certified birth certificate issued by the county or state*
    • Consular Report of Birth Abroad
    • Naturalization Certificate
    • Certificate of U.S. Citizenship
    • Provide the original and a photocopy of the front and back (single-sided copies only)
  3. Proof of Identity, such as:
    • Valid Driver's License
    • Current Government ID (city, state, or federal)
    • Current Military ID
    • Provide the original and a photocopy of the front and back (single-sided copies only)
    • Minor applicants must provide proof of relationship to parent(s), identifying on their behalf (e.g., original and copy of birth certificate or court documents)
  4. One Valid Passport Photo
  5. Two separate payments are required as follows:
    • Payment One: Application Fee, payable to "U.S. Department of State"
      • Personal check, money order, or cashier's check ONLY
      • Credit and debit cards are not accepted.
    • Payment Two: Execution Fee, payable to City of Chino Hills
      • Personal check, money order, cashier's check, or credit/debit card ONLY
  6. Children under the age of 16 must be accompanied by BOTH parents.

Passports Previously Issued to Minors

A passport that was issued to a minor is NOT renewable by mail and should be processed at an acceptance facility using Form DS-11 with both the previous passport and the birth certificate (original and photocopy.)

Processing Time

Regular Service currently takes up to 6 weeks.
Expedited Service currently takes up to 3 weeks.

*A certified birth certificate has a raised, embossed, impressed, or multicolored seal. The registrar’s signature and date must be visible. Abstracts, which do not include complete parental information, are not acceptable for passport purposes. Short-format abstract certificates from California are never acceptable.

Convenience Fee for Credit/Debit Card Payments
A non-refundable 2.5% convenience fee is assessed on all credit/debit card transactions paid to the City of Chino Hills for passport-related services, which may include additional fees such as photo and/or copy fees.

**When applying for both the book and card at the same time, the $35 execution fee made payable to the City of Chino Hills is only charged once.
